Good in a Crisis is a reported nonfiction project by Christopher T. Perkins, BSN, RN, about the human cost of working in healthcare.

This questionnaire is an invitation to describe your experience in your own words. It is not an academic research study, clinical assessment, employment complaint process, or mental-health service.

Participation is voluntary. You may skip any question that is not required and may leave before submitting.

Complete five main sections. Three optional deeper sections are available, and every question in those sections is optional.

Your permission matters

Near the end, you choose whether your responses may be quoted or paraphrased and how—or whether—you may be identified.

Privacy matters

Do not include patient names, dates of birth, medical-record numbers, exact dates, room numbers, photographs, or other identifiable patient details.

Content note

The questionnaire includes topics involving trauma, death, suicide, workplace violence, grief, mental health, substance use, and other difficult experiences. You control how much you answer.
